Locate Docs

Locate all relevant documents to present at the Consulate. Examine all your documents carefully before submitting your application.

Verify

Photocopies or copies certified by a Notary Public will not be accepted. Photographs must be taken at a PROFESSIONAL photo studio. Instant photos are not acceptable.

Submit

Applicants MUST submit a legibly completed application form in full. On PAGE TWO Applicants must also sign declaration, in signature box . BOTH signatures MUST be identical.

Passport Info

The Passport, Immigration & Citizenship Agency (PICA) regulations and guidelines govern all applications for passports and visas. The Consulate facilitates the application process, but PICA is the final authority. An applicant for a passport or a visa should present all required documentation at the time of the application. Applicants MUST submit (a) legibly completed application form in full which must be signed in all applicable places; and (b) accompanied by requisite documentation and identification.

PASSPORTS

Processing period:

Processing time is based on final approval of all requirements by the Passport, Immigration & Citizenship Agency (PICA) in Jamaica.

Mailed - in – 14 weeks from approval of Application.

Walk - in - 10 weeks

Payments:

Mailed-in (a) money order or cashier’s cheque (check) payable to the Consulate General of Jamaica - Miami

Walk-in (a) money order or cashier’s cheque (check) payable to the Consulate General of Jamaica – Miami; or (b) cash – exact fee only

Cancellations:

Cancellation of an application must be requested in writing ( no exceptions) and signed by the applicant. In the case of minors, this is to be done by the parent/guardian who applied on the minor’s behalf.

Refunds:

Refunds are not automatic and are only payable by the Consulate if the Application is not yet submitted to the Passport Immigration and Citizenship Agency (PICA) or dependent on the stage of the process of an application. Requests for refunds must be done in writing and signed by the applicant. In the case of minors, this is to be done by the parent/guardian who applied on the minor’s behalf.

Mail and Consular fees will be deducted on processing refunds.

Your passport will be returned to you via US Postal Service (Express Mail).

US Postal Service will not deliver to P.O. Box numbers and a regular mailing address is required with Street Address &/or Apt Number, and ZIP Code where applicable, for the return of your documents. 

All supporting documents should be original and not photocopied.

The following are instructions for the completion of the Jamaican passport application form. Please pay close attention to the following:

1. Ensure the applicant's signature is affixed in the CENTER of the signature box at the top of page 2.
2. Married men or women are to complete Section B.
3. A Consulate Official will indicate the document number and date of issue of all submitted documents in Section K.
4. Complete Section C for children under 18 years old. The parent's or legal guardian's signature must be placed on the line in Section C. The parent giving consent MUST be on the minor's birth certificate.
5. The parent or legal guardian is required to submit a valid state issued valid picture identification.
6. Tick the appropriate box in Section E and affix your signature to the signature line.
7. Section F must be completed with the name and relationship for your emergency contacts.
8. Section G must be signed by a Notary Public, Jamaican Honorary Consul or Jamaican Diplomatic/Consular Officer. If the form is being signed by a Notary Public, please note that the date required in Section G is the date on which the application was witnessed and not the date of validity of the commission of the Notary Public.
9. Persons who are required to wear headgear for religious purposes, must complete Section H. 

 Photographs

Please note professional photography service is available on the building at 44 W. Flagler should there be any challenge accessing this service before arrival at our office.

Signature

On page two of the application form, there is a rectangular signature box at the top of the page. Please sign in the middle of the box. Signature MUST NOT touch any of the lines. Applicants must also sign declaration. BOTH signatures MUST be identical.

Birth Certificates

The Registry of Birth document is not a birth certificate and is not acceptable.

A certified copy of your birth certificate can be obtained from the Registrar General’s Department, www.rgd.gov.jm

Mailed In (Compliant) - 14 Weeks

 IMPORTANT Notes regarding Vital Records Documents, Photo Identification & Photographs

Examine all your documents carefully before submitting your application. Make sure that they are the original issued by the proper authority* (photocopies or copies certified by a Notary Public will not be accepted – no exceptions). Documents will be returned on completion of application process along with passport or visa.

Proper authority: All supporting documents (birth, death, marriage, divorce, name change, adoption, et cetera) must be CERTIFIED copies issued by the Official records office in the district where the event occurred, e.g. Registrar General’s Department, Bureau of Vital Statistics/Records, County Court, and must bear the original seal / stamp and a signature of the issuing authority.

Female married applicants should be aware that the acceptable marriage certificate must be one obtained from the Registrar General’s Department (if married in Jamaica) and if married outside of Jamaica, the document is one issued by (a) the Vital Statistics or Vital Records Office in the State (USA) where the marriage took place; or (b) the office where the marriage licence was obtained, such as a county court or marriage license bureau/registry (USA); or (c ) appropriate Vital Records office in the country where the marriage took place outside of the United States.

A Foreign Language record must be accompanied by an official translation of the document and must bear the original stamp/seal/signature of the translating authority.

Court documents such as divorce judgments, adoption orders, name change decrees, must be certified by the court that issued the document and include the official's title and his/her signature which must be legible and a record number (such as a Book/Page or Volume/Folio number, et cetera) and the date on which the certified document was issued. 

 First-Time Applicant Minor ( persons under 18 yrs old ) :

1. A Completed passport application form (Notarized at Section G)
2. A Completed “First Time Application Form”
3. Two (2) identical non digitized color photographs, one of which should be certified by the Notary Public who certified the application
4. Original Birth Certificate or Original Certificate of Citizenship or Adoption Register
5. Parent’s valid ID such as passport, Drivers Licence, National Voters ID (a notarized copy of ID may be submitted if being sent by mail)
6. The parent / legal guardian submitting the application is required to complete section C of the application form and also sign at Section E
7. Parent’s signature on application form must be consistent with (match) the signature on the ID being submitted
8. Legal Guardians are required to submit a Court Order along with their ID or a notarized letter of consent from the parent(s) along with a notarized copy of their ID
9. Name change document e.g. Marriage Certificate if mother’s name on Birth Certificate differs from name on ID
10. Children 12 years and older must sign in the signature box at the top of the page without touching any of the borders of the box
11. All first-time applicants must apply in person

FEE : U$100.00 - First Time Minor (Payable by Money Order to: Consulate of Jamaica, Miami – Personal Cheques are not acceptable)

Minor Passport Renewal ( persons under 18 yrs old ) :

1. A Completed passport application form (Notarized at Section G)
2. A Current passport
3. Two (2) identical non digitized color photographs, one of which should be certified by the Notary Public who certified the application
4. Original Birth Certificate or Original Certificate of Citizenship or Adoption Register
5. Parent’s valid ID such as passport, Drivers Licence, National Voters ID ( a notarised copy of ID may be submitted if being sent by mail)
6. Parent’s signature on application form must be consistent with (match) the signature on the ID being submitted
7. The parent / legal guardian submitting the application is required to complete section C of the application form and also sign at Section E
8. Legal Guardians are required to submit a Court Order along with their ID or a notarized letter of consent from the parent(s) along with a notarized copy of their ID
9. Mother’s Name Change document e.g. Marriage Certificate if mother’s name on Birth Certificate differs from name on ID

Minor Passport Replacement ( persons under 18 yrs old ) :

1. A Completed Passport Application Form
2. Police report or Lost Passport Form and Lost Passport Form Two
3. Two (2) identical non digitized color photographs, one of which should be certified by the Notary Public who certified the application
4. Original Birth Certificate or Original Certificate of Citizenship or Adoption Register
5. Parent’s valid ID such as passport, Drivers Licence, National Voters ID ( a notarised copy of ID may be submitted if being sent by mail)
6. Parent’s signature on application form must be consistent with (match) the signature on the ID being submitted
7. The parent / legal guardian submitting the application is required to complete section C of the application form and also sign at Section E
7. Legal Guardians are required to submit a Court Order along with their ID or a notarized letter of consent from the parent(s) along with a notarized copy of their ID
8. Mother’s Name Change document e.g. Marriage Certificate if mother’s name on Birth Certificate differs from name on ID

FEE : U$150.00 - Minor Replacement (Payable by Money Order to: Consulate of Jamaica, Miami – Personal Cheques are not acceptable)

Processing time for passport applications: Approximately 10 weeks for in person applications; approximately 14 weeks for applications received by mail
Applicants submitting their own applications at the Consulate are not required to have forms and photographs notarized. 

 First Time Applicants: Adults

1. A Completed passport application form (Certified at Section G)
2. A Completed “First Time Application Form”
3. Two (2) identical non digitized color photographs, (One photograph must be certified by the same official who certified the passport application)
4. Original Birth Certificate or Adoption Register and letter advising how you arrived in the United States (for persons born in Jamaica) or  Citizenship Certificate for persons born overseas
5. Proof of name change. If name has been changed, submit either (Original Marriage Certificate, Original Deed Poll, Original Divorce Decree or Court Order)
6. Government issued ID such as Driver’s Licence or Passport
7. All first-time applicants must apply in person

FEE: U$130.00 - First Time Adult (Payable by Money Order to: Consulate of Jamaica, Miami – Personal Cheques are not acceptable)

Adult Passport Renewal

1. Completed passport application form (Certified at Section G)
2. Current Passport
3. Two (2) identical non digitized color photographs, ( One photograph must be certified by the same official who certified the passport application).
4. Original Birth Certificate / Adoption Certificate or Original Jamaican Citizenship ( as applicable )
5. Proof of name change. If name has been changed, submit either ( Original Marriage Certificate, Original Deed Poll, Original Divorce Decree or Court Order).
6. Notarized copy of valid Government issued Photo Identification (Example: Drivers Licence, Voter Identification) – If visiting the Consulate please take the original document
7. An applicant whose last passport was issued as a notarized copy of valid Government issued Photo Identification (Example: Drivers Licence, 8. Voter Identification) If the applicant does not have an ID, the parent should complete a Declaration of Identity Form and submit along with a notarized copy of their ID. (If visiting the Consulate please take the original document)

Adult Passport Replacement ( Lost or Stolen Passport )

1. A Completed passport application form (Certified at Section G)
2. Police Report or Lost Passport Form and Lost Passport Form Two
3. Two (2) identical non digitized color photographs, (One photograph must be certified by the same official who certified the passport application).
4. Original Birth Certificate / Adoption Certificate or Original Jamaican Citizenship (if applicable)
5. Proof of name change. If name has been changed, submit either (Original Marriage Certificate, Original Deed Poll, Original Divorce Decree or Court Order).
6. Notarized copy of valid Government issued Photo Identification (Example: Drivers Licence, Voter Identification) – If visiting the Consulate please take the original document

FEE: U$210.00 - Adult Replacement (Payable by Money Order to: Consulate of Jamaica, Miami – Personal Cheques are not acceptable)

If you have an emergency and do not have a valid passport, you may apply for an “Emergency Certificate”. The Emergency Certificate is used primarily for travel to Jamaica and may be issued in cases where there is an unforeseen and immediate need to travel. Situations involving illness, death, loss of passport while traveling, or unexpected business travel would be considered an emergency. It is recommended that proof of the emergency be submitted with the application.

It should be noted that you will be required to complete a renewal or replacement passport application at the time of application for the Emergency Certificate.

Applications - Sent in By Mail;

- A Completed Passport Application Form
- Expired Passport
- Birth Certificate
- Marriage or Divorce Decree as Applicable
- Valid Photo I.D. (e.g. Driver License, Green Card)
- Completed Online Emergency Travel Document Form
- Two Passport sized Photos - (1 must be notarized)
- A printed copy of the Confirmed Travel Itinerary
- Money Order in the amount of US$110.00 made payable to the Consulate General of Jamaica - Miami
- All mailed-in applications must be signed in all relevant locations and all documents submitted MUST be ORIGINALS
- Processing time: 4 - 7 business days

If you have an existing Jamaican Passport Application and require an Emergency Travel Document for immediate travel;

Only the following is required:

- A Completed Passport Application Form
- Completed Emergency Travel Application Form,
- Two (2) Photos, and
- A printed copy of the Confirmed Travel Itinerary.

For Applications - In Person at the Consulate, the following is required;

- Completed Passport Application Form
- Expired Passport
- Birth Certificate
- Marriage or Divorce Decree as Applicable
- Valid Photo I.D. (e.g. Driver License, Green Card)
- Completed Online Emergency Travel Document Form
- Two Passport sized Photos - (1 must be notarized)
